STEP 1: Create MyAdoptionPortal.com account

‍DO NOT FILL OUT THIS FORM UNLESS YOU HAVE HAD A PERSONAL VISIT WITH OR A PHONE CONVERSATION WITH A REPRESENTATIVE OF Adoption Life. CONTACT US to set up that visit.

NOTE: The form on this page is not an application for adoption. It simply enables us to send you a username and password so that you can begin the application process.

STEP 2: receive username and password for MyAdoptionPortal account

A username and password will be emailed to you. If you do not see the email in your inbox, please check your spam folder.

STEP 3: log in to MyAdoptionPortal.com account to fill out online application

Existing Families

‍If you have completed a previous adoption with Connecting Hearts or Adoption Life and are planning to adopt with us again, please contact Meghan Anderson at [email protected]. We will review your previous account with you and connect you with the application. This will allow us to make sure that your current files are imported properly.
